EXCERPT FROM PARENTS LIST JANUARY 1998 (#4) Date: Wed, 7 Jan 1998 10:29:06 +0000 Subject: Labour Compared to your birth labour stories, mine sounds like a labour straight from hell. I had epidural (ok, I'm a chicken as far as pain is concerned). Kieran who was almost 4 kg at birth was stuck in the birth canal for close to an hour until he was pulled out using forceps. You should have seen the "war marks" on his face because of the forceps. To top it all off, after Kieran came out, I had pph (post partum hammerrhage (sp??)). I lost so much blood that I drifted in and out of consciousness and at one point, they couldn't find my blood pressure. My husband who was there was almost freaking out :-) I had 2 pints of blood for transfusion and was knocked out for almost 24 hours. As for Kieran, he survived the whole ordeal much better than me. Despite the epidural, he wasn't sleepy. The bad news for me is that my gynae said that the pph is likely to happen again for this second birth. Enough moaning for now... About the breathing exercises, I took an antenatal class where they taught various breathing techniques for different stages of labor (Lamaze), but I don't think it will be too useful for me. I have read the book Natural Childbirth the Bradley Way, where they recommend simple abdominal breathing, which I think is more helpful (and easier to remember at least). I really wanted to take a Bradley class, but I couldn't find any in Singapore, so I settled for the book instead. The relaxation and pushing exercises I learned in the class were good though. Jacqueline, did NUH charge extra for foreigners? If you're PR then ignore the question. That was the reason I switched from government to a private hospital. Even the first one actually. The midwife put the baby next to me and that made it easier to bf the baby. I gave birth at SGH for my first baby and at KKWCH for my other two babies. I found the nurses friendly and very pro bf. They encouraged bf from the start and were patient and willing to teach me. They discouraged bottle feeding or supplementary bottles since I wanted to bf my baby totally. Only the first night, the baby was given a supplementary bottle, but fed from a cup. So that bf would be easier since the baby wouldn't get used to a rubber nipple. The most expensive package which is S$4,300 ( that's the charge for foreigners) includes the following:- standard pre-natal checks by consultants; ob's delivery charges (consultant); first ob scan and post-natal check; accomodation (single room- 2 days for normal assisted deliveries and 5 days for c-section) and daily treatment; one time use of operating theatre; newborn's first hepetitis B and BcG vaccination; 1st screening by pedi; and G6PD test and thyroid screening for newborn. This is also the package that I opt for. My ob is from Hong KOng and he is the best ob I've ever met but he'll be heading for a Cambridge genetic counselling course for 2 years. Nonetheless, there are lots of good consultants in NUH. Let me know if u need to book for an appointment. The good thing about NUH is that it's affiliated with the Medical School of the national univeristy here and resources are plentiful. Sarah had rash with using cloth, really. Probably b/c I don't like mess or leaks so I used diaper covers. Rash went away miraculously when I changed to disposables. But Sarah did go on to have a greater tendency for diaper rashes as compared to Daniel. Both the kids have sensitive skin with tendency to eczema. Daniel doesn't get it quite so bad, and I like to think it's b/c I bf him. Sarah ff after 1 mth. I still have a tiny freezer stash. Have not been pumping since Mid-December. Too tired of it already. I just work my schedule around my feedings. Rita & Guek Har & May WAng use the PIS. Have gone back to work and am trying very hard to balance work and family. After the Christmas week break and returning to working and pumping, it happened again - those plug ducts, yikes ! It was only day 1 and I returned home with a painful left breast and day 2 I developed a fever (37.9 C). The doctor sent me home with 2 days MC :-( I guessed it is right that no pump is as efficient as the baby. I am nursing the baby as much as possible from the left breast and now my breasts looked lopsided. We are doing great and Jun Wei weighs 6.4 kg (!) at his 2 months checkup. My MIL who visited us during Christmas time did not even mention about formula supplement (she "complained" over the phone a few times about baby not getting enough just from bm). Now he wants to get off my arms and run like for eg, to see the Polar Bears! We have a "Friend of the Zoo" Family Pass. It only makes sense since we stay in Yishun - quite near the zoo. We visit often. But we only got the family pass b/c Sarah was already past 3yrs old when our last passes expired. Before that, we survived on a Dual Pass (2 adults) since our children were below 3yo and get in FREE. One advantage of the Family pass though is that Grandparents / Maid can substitute for a "missing" adult. With the Pass, we only take short visits each time. I will "bargain" with Sarah. Today, only see seals, polar bear, elephants. Or today, only go to the children's zoo & sit train. It's easier on us & the kids. It's difficult to have the kids out the WHOLE day at the zoo trying to cover the whole zoo. Dang-gui is suppose to help with blood circulation and since breast milk is called "white blood", it should help too. I would suggest u only start using cloth diapers after the 1st mth. In the inital mth, we're going thru something like 12 diapers a day - almost a bm every hr. This should give u enough time to rest (u definitely need the rest for the 1st mth!) and to monitor her habits. Am amazed too that Lai Chang can last so long. Guess I'll take it one step at a time setting incremental targets. Not easy as Sheralyn now is more interested in her surroundings. Got to look for a good location. I use both cloth and disposables, the cloth diapers are mostly passed down, so I did not buy any. Not much difference in terms of quality, they all get the job done ! I think it is too costly, not to mention not environment friendly, to use disposables since both my kids poop ever so frequently in the first months. 3. For my first baby, I went through about 6 hours of labour and it was *very* painful. But I did not use any drugs except for the gas. Second time round, it was almost painless and baby was out in less than 2 hours. 4. Mmm I am not sure about the breastfeeding rate in Singapore, anybody with that figure ?? Most of my colleagues stopped before they return to work which is usually 8 weeks post partum. But I also know of a few who express their milk at work. No I haven't seen anybody bfeed in public before (OK discount those times when I visit new moms). They must be very good at hiding it I think. For me, I rarely breastfeed in public, except in the clinic or in the car. DH is *very* worried about me exposing myself. The bf-friendly hospitals will ask you to, like at Mt Alvernia where I delivered #2 - Daniel. At Eastshore where I delivered #1 - Sarah, they took so long to give baby to me even when I asked to bf her. They kept asking me to wait - like as if I was such an anxious 1st-time parent. Made us a little *embarrased*. And at Eastshore, even though I requested (even had a letter attached to my admission report) TOTAL bf, when they brought Sarah to me later, I saw on the feeding chart that they had given her a bottle of FORMULA! And I think from Jude Constable's remarks earlier (she delivered in ESH too), Eastshore hasn't improved very much in that respect! AS for Expelling the Placenta, most Gynae here in Singapore practice injecting a hormone right after delivery to assist in (almost) immediate delivery of the afterbirth. For my case, I was seeing a private specialist. It's easier to get my checkup (can go at nite or weekends) and he did an ultrasound everytime I went (monthly basis). When I transferred to KKWCH at the eight mth and requested for an ultrasound, the ob. told me that there's no need for it if the ultrasound at 5th mth is ok. Also the ultasound is done at another department, all the ob. has is just a machine to check the heartbeat rate. He uses a measuring tape (the same one my mum uses for tailoring) to measure the size of the baby. Booking an appt. with my ob. is tedious. Such a long queue and only available for 2.5 days per wk. Both Daniel & Sarah had such fun! They looked at planes, also looked throught the glass windows to see people walking around inside the "restricted" areas, crawled under the things that hold telephone books. And they have these spherical things where you can donate coins. The coins will then circle round & round before dropping into the bin. The kids really enjoy that! And there's lots of space to RUN! But other than morning sickness the 1st 4 to 4.5mths, after that, I feel just GREAT! Except with Daniel I was studying & taking exams from my 6th month until 9th mth. That, coupled with demanding Sarah. My gynae said at 8mths, I was having just a bit too little amniotic fluid and needed to rest & relax more. I specifically asked my ob not to do an epis, and even wrote it down on paper, but got one anyway after pushing for about 10 minutes because she said we had to get baby out fast as its heart rate was dropping with every push. ;( Till this day, I don't know if that epis was really necessary, but a labouring woman at the pushing stage isn't likely to argue with her ob, and she probably knew it. I know that some mothers who managed to bf until 4 to 5mos finally quit at around 5 to 6mos, citing the reason as "baby easily distracted, doesn't want to bf anymore, so I stop bf'g. It's so sad that baby doesn't need me anymore". It's a stage babies go through. You'll just need to go to a familiar / quiet place to nurse. At the age of 5mos, Daniel would nurse if I brought him to my bedroom & nurse him while sitting on my bed. That's b/c I bf him there most often. If I went to anywhere else, like the sofa in the guestroom, he would be so distracted by the "strange" room that he wouldn't bf at all! And now, he wouldn't bf if he isn't also hugging his "bear bear" at the same time - the same one that Sarah is sooo attached to.
